申请AQSIQ证书需要提供以下材料
Basic Documents Required For AQSIQ New Registration

1、在线打印的申请书 Application Form(must be printed on-line of AQSIQ system after being registed)

2、两份经过公证的商业文件和税务登记文件的公证件原件，以及中文翻译件；
Double sets of the orignal notarized business and tax registration documents(either of which should include company's name, legal representative's name and registered address) with their Chinese translation version. You can send me one set firstly, and after I submition you can take your time to do another one then send to me, or you can do two sets at the same time, it depends on which way is convenient for you.

3、标明尺寸的固定办公场所平面图,有加工场地的，需同时提供加工场地平面图,能全面展现上述场地的实景视频和8张以上照片，每张照片需标明是属于什么部门；平面图需标明测量单位和面积：
The fixed office plan view/layout marked with Size and Measuring Unit, if you have a processing yard,
please provide it as well. If you don't have it then no neccessar to provide.
More than Eight photos which can fully display your internal office, and please mark each department's name you take picture of, I need to know what is the dept's name you taking picture of. You can match each name on each picture. And please mark the measuring unit (eg.feet or meter) and the size of the floor plan.
4、放射性检测仪器的照片,合格证书或者校准证书及中文翻译件
Pictures of the radiation detector,
the calibration certificate or qualified certificatewith its Chinese translation.
which can show us the radiation detector is well operated.

5、ISO9001质量管理体系或者 RIOS体系等认证证书彩色复印件及中文翻译件;
The ISO9001 quality management system or RIOS system and other relevant
authentication certificates' color photocopy with its Chinese translation.
